About the role
Since 1985, Albuquerque Health Care for the Homeless has dedicated its mission exclusively to providing services to people experiencing homelessness. AHCH provides a distinctive continuum of integrated care through outreach and site-based services to address the health-related causes and consequences of homelessness. Our vision is to live in a world that is just and without homelessness. We believe that homelessness is an issue that can be solved through access to quality health care, adequate and affordable housing, and a living wage.
Responsibilities
- Assess individual needs of clients
- Create individualized service plans with direct input from client in accordance with CCSS requirements
- Work in an integrated manner with other AHCH programs to optimize client care
- Refer client to appropriate programs and agencies for coordination of wrap-around services in the community
- Participate in client chart audits for peer review
- Provide direct service to clients with 60% of contacts conducted in home visits/community visits
- Perform duties in a trauma informed and client centered manner
- Ensure accurate and appropriate documentation of all client contacts in Electronic Health Record (EHR) within timeframe required by AHCH procedure and billing standards
- Attend program-specific meetings and trainings including clinical supervision with Behavioral Health Clinical Director
- Attend organizational meetings and trainings
Qualifications
- Ability to work in a client centered, trauma informed, and harm reduction manner to assist clients in meeting the goals that they develop
- NM driver's license (or the ability to obtain an NM driver's license within 30 days of start date) and clean driving record
- A bachelor’s degree in a human service field from an accredited university and one year relevant experience with the target population
- OR An associate’s degree and a minimum of two years of experience working with the target population
- OR A high school graduation or general educational development (GED) test and a minimum of three years of experience working with the target population
- CCSS certification preferred
- Experience in trauma-informed care and harm reduction models preferred
- Bilingual or Spanish speaking preferred
