Community Support Team Case Manager

Responsibilities

• Provides direct service to CST clients through mental health rehabilitation services and supports.
• Identifies and uses adaptive and compensatory skills, natural supports, and community resources.
• Works with CST team members to document services according to Rule 132 and organizational policies.
• Evaluates referrals and facilitates placement into CST program.
• Aids clients in achieving treatment goals, maximizing independence, and accessing community resources.
• Coordinates with other community agencies and organizations.
• Facilitates transitions in living arrangements and provides intensive outreach.
• Participates in program meetings and trainings.

Qualifications

• Bachelor's degree in psychology, social work, or related field from an accredited college or university.
• Experience working with clients in a behavioral health setting.
• Substance abuse treatment experience preferred.
• Case Management experience preferred.
• Bilingual (English/Spanish) preferred.
