About the role

Community Support Services is seeking a Case Manager to provide comprehensive support services to individuals in need.

Responsibilities

  • Assess client needs and develop individualized service plans
  • Coordinate services and resources to meet client needs
  • Provide direct case management services including advocacy, problem-solving, and resource coordination
  • Maintain accurate and timely documentation of all services provided
  • Participate in regular team meetings and training sessions

Requirements

  • Bachelor’s degree in Social Work, Psychology, or related field
  • Minimum 2 years of relevant experience in social services or related field
  • Licensure as a Social Worker or equivalent certification
  • Strong interpersonal and communication skills
  • Ability to work independently and as part of a team

Qualifications

  • Proficient in Microsoft Office Suite
  • Experience with case management software
  • Excellent organizational and time management skills

Skills

  • Strong problem-solving and critical thinking skills
  • Excellent written and verbal communication skills
  • Ability to build and maintain positive relationships with clients, colleagues, and community partners

Pay

Salary range: $40,000 - $50,000 annually

Schedule

Full-time position, Monday through Friday, 8:00 AM - 5:00 PM
