About the role
Community Support Services is seeking a Case Manager to provide comprehensive support services to individuals in need.
Responsibilities
- Assess client needs and develop individualized service plans
- Coordinate services and resources to meet client needs
- Provide direct case management services including advocacy, problem-solving, and resource coordination
- Maintain accurate and timely documentation of all services provided
- Participate in regular team meetings and training sessions
Requirements
- Bachelor’s degree in Social Work, Psychology, or related field
- Minimum 2 years of relevant experience in social services or related field
- Licensure as a Social Worker or equivalent certification
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
Qualifications
- Proficient in Microsoft Office Suite
- Experience with case management software
- Excellent organizational and time management skills
Skills
- Strong problem-solving and critical thinking skills
- Excellent written and verbal communication skills
- Ability to build and maintain positive relationships with clients, colleagues, and community partners

Pay
Salary range: $40,000 - $50,000 annually
Schedule
Full-time position, Monday through Friday, 8:00 AM - 5:00 PM
