CASE MANAGER & COMMUNITY SUPPORT SPECIALIST

About the role

Catholic Charities Family and Community Services (CCFCS) is a regional agency dedicated to serving the community for over 100 years. As a Case Manager & Community Support Specialist, you will play a vital role in providing support and advocacy to clients.

Responsibilities

• Maintains a caseload of 8-10 clients from the Marketplace Senior Apartment 55+ program
• Conducts intake assessment and services to clients referred to Marketplace Apartments, self-referrals, and partnering agencies
• Completes required intake paperwork and screenings
• Creates and implements comprehensive, person-centered, trauma-informed and solution-focused plans and goals
• Supports clients in accessing resources such as housing, education, employment, and legal issues
• Ensures client needs are met through coordination with other stakeholders
• Communicates with service providers, community partners, and criminal justice agencies
• Facilitates vehicle maintenance and repairs
• Maintains regular contact with clients to coordinate benefits and address treatment needs
• Documents client interactions and progress
• Participates in relevant Agency meetings and ongoing professional development

Requirements

• Education: Bachelor’s Degree in Social Work, Human Services, Criminal Justice or related field preferred
• Experience: Minimum two years of experience with case management and community outreach in areas such as substance use disorder, geriatrics, mental health, and refugees
• Physical Demands: Frequent sitting, standing, walking, talking, hearing, using hands to type, and performing light lifting
• Other: Valid and clean NYS Driver’s License and PPD screening available

Qualifications

• Knowledge of Medicaid, SNAP, Social Security, and legal systems helpful
• Ability to work in various community locations and settings
• Compliance with applicable laws and regulations
