About This Role

The purpose of the Community Services Specialist position is to assist community members in applying for benefits (Medicaid, Medicare, Food Stamps, Cash Assistance, Social Security, Disability, Florida KidCare, etc.) via the state of Florida's web portal (ACCESS) as well as assisting community members in the preparation of electronic and paper applications to establish eligibility and enrollment in coverage through the Health Insurance Marketplace and potentially qualify for an insurance affordability program. The Community Services Specialist will also serve as a community liaison for participants by connecting them with viable services offered by Evara Health and throughout Pinellas County.

What You'll Do

  • Client Support & Resource Navigation: Assist clients with applications for health insurance, social services, and other assistance programs through one-on-one counseling. Assess client needs, provide referrals, and advocate on behalf of clients to help resolve resource-related challenges.
  • Care Coordination & Follow-Up: Follow up on referrals from care team members and track clients' progress toward accessing services and benefits. Maintain accurate documentation and progress updates in the electronic records system.
  • Community Outreach & Education: Promote Community Services programs within Evara Health and throughout the community through outreach events, local organizations, and stakeholder partnerships. Provide education on ACCESS, the Health Insurance Marketplace, and available community resources.
  • Food Assistance & Pantry Operations: Enroll patients in the Food Rx Program and assist with vouchers and access to appropriate food resources. Manage daily food pantry operations, including inventory monitoring, ordering, stocking, and organization.
  • Patient Access & Operational Support: Register and schedule patients in the EMR and maintain required Evara Health EMR certification. Complete required training and provide coverage at other Evara Health locations as needed.

Education and Experience

  • High School Diploma or GED certificate; some college preferred
  • Minimum one (1) year of experience as front office staff in a healthcare setting and/or customer service preferred
  • Ability to navigate and enter data into an electronic health record

Certification Requirements

  • EMR Certification
  • CAC Certification
  • Food Handling Certification
