Community Resource Specialist | PACE Program

About the role

Helps older adults remain safely and independently in their homes by connecting them with healthcare services, financial assistance, and community resources. The role serves as a link between participants, families, healthcare providers, and local agencies to ensure seniors receive the support they need.

Responsibilities

• Helping participants access housing, transportation, food assistance, utility support, and public benefits such as Medicaid and Medicare
• Working closely with nurses, social workers, therapists, and other members of the interdisciplinary PACE team to coordinate care and advocate for participants' needs
• Educating families about available services
• Maintaining participant documentation
• Providing emotional support during challenging situations

Qualifications

• High school diploma or equivalent (some positions may require an associate or bachelor's degree in human services, social work, healthcare, or a related field)
• Experience working with older adults, healthcare, social services, case management, or community outreach
• Knowledge of community resources, Medicaid, Medicare, and public assistance programs
• Strong communication and interpersonal skills
• Ability to coordinate services and work collaboratively with an interdisciplinary healthcare team
• Organizational and documentation skills for maintaining participant records
• Valid driver's license and reliable transportation may be required for community visits
• Ability to maintain confidentiality and work with sensitive participant information

Preferred

• Bachelor's degree in social work, psychology, human services, gerontology, or a related field
• Previous experience in a PACE program, senior services, behavioral health, or healthcare setting
• Experience assisting clients with benefits applications and care coordination
• Bilingual communication skills
• Familiarity with electronic medical records or healthcare documentation systems
• Crisis intervention or advocacy experience
• Knowledge of aging-related programs and long-term care services
• Strong networking skills and established connections with local community agencies
