Community Outreach Specialist - PACE

Responsibilities

  • Raise and implement an outreach strategy for the areas served by BCSC PACE to engage potential enrollees and their family caregivers.
  • Collaborate with communications, outreach, and enrollment staff, and public and community relations efforts, including materials development, with the BCSC PACE Marketing Director, and Corporate LAJHealth Branding and Marketing Department.
  • Perform community outreach activities including presentations, exhibits, and one-on-one contact with community groups, and coordinate special events for BCSC PACE.
  • Establish new relationships with community groups (e.g., Senior Centers, Food banks, Chambers of Commerce, Clergy, etc.) and foster and nurture these relationships with help from the BCSC PACE Marketing Director to generate more referrals to BCSC PACE.
  • Create and coordinate new market research efforts to analyze the effectiveness of current marketing efforts, as well as to assess improvements required to increase market penetration.
  • Maintain and share a database of community service information.
  • Manage the “Top of the Funnel” enrollment process of Community Awareness of BCSC PACE and perform all preliminary interest intake functions in coordination with other BCSC PACE marketing staff, working closely with the Center Manager, the Marketing Enrollment Team to assure appropriateness of candidates for the program.
  • Field direct and indirect referrals discovered through Community Outreach over to Enrollment Specialists to determine initial eligibility and qualification of candidates for enrollment.
  • Work closely with the BCSC PACE Marketing Director, Center Manager, and IDT on any Group Tours, Open Houses, or Outreach meetings larger than 12 Seniors at a time to ensure that potential participants are greeted and oriented to the PACE Center during visits.
  • Spread Awareness of NPA PACE messaging and the connecting of PACE Life-Changing Full Care, with the vulnerable and frail Seniors living within our Service areas.
  • Become the Subject Matter Expert (SME) of all options and choices in Healthcare available in our communities to ensure Higher Hit-Rate of closing for our Enrollment Specialists.
  • Support BCSC PACE Marketing Director and Marketing Department staff to insure team Census Growth success.
  • Affirm that all marketing and enrollment activities comply with state and federal regulatory requirements including but not limited to PACE.
  • Maintain and comply with the Participant Enrollment Agreement Terms and Conditions, marketing materials, presentations, and DHCS and CMS policies and procedures.
  • Comply with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions to the Center Manager. Take necessary measures to ensure a safe environment for oneself, co-workers, contractors, participants, visitors and others.
  • Effectively collaborate with staff peers and contractors to meet BCSC PACE goals and further success.
  • Comply with all policy and procedures of BCSC PACE.
  • Demonstrate proficiency in delivering age-specific care.

Qualifications

  • Degrees and Diplomas: Bachelor’s Degree in Marketing, Public Relations, Communications or related field, or equivalent experience required.
  • Certificates, Licenses: DHCS Certification as a marketing representative within 90 days of hire. Verification of completion of CPR (BLS) and first aid training.
  • Experience: Minimum of two years’ experience in sales-oriented senior health services marketing or public relations. Supervisory experience preferred. Experience in increasing market share and/or rolling out new programs preferred.
  • Training: Must possess knowledge of how PACE works or be willing to be trained on the full spectrum of what it is and how BCSC PACE compares to other PACE Programs that are available in the community. Demonstration of strong community networking contacts.
  • Skills/Aptitudes: Desktop publishing, word processing and audio-visual (AV) preparation experience. Comprehensive understanding of the current healthcare environment, including managed care. Proficiency in market analysis.
