Community Residence Counselor - Appleton

About the role

Under the direction of the Program Manager, the Community Residence Counselor provides direct patient care and participates in the therapeutic milieu within the defined scope of practice. This role is based at Appleton, a step-down residential program for adults with serious mental illness, designed to help them stabilize, build skills, and transition back to independent living.

Responsibilities

• Provides direct patient care in accordance with the current treatment plan, under the supervision of the Nurse Manager, Clinical Coordinator, and Charge Nurse.
• Provides and supervises community activities according to patient needs.
• Remains awake and alert at all times while on the job.
• Assists patients in maintaining and keeping various appointments.
• Completes clear, concise, and timely documentation and shift notes.
• Establishes helpful relationships with patients while maintaining professional boundaries.
• Participates in patient rounds, status checks, or teams per program guidelines.
• Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
• Works effectively to manage the therapeutic milieu.
• Provides patients with direction and supervision as needed.
• Follows established hospital and program policies, procedures, and protocols.
• Communicates information regarding the patient milieu/therapeutic environment to appropriate staff in a timely manner.
• Communicates observations related to medication side effects and physical and emotional changes to appropriate medical staff in a timely manner.
• Encourages patients in their activities of daily living.
• Assesses sleep patterns and assists patients in developing more appropriate sleep cycles, notifying the clinical team as necessary.
• Completes mandatory annual in-service requirements, including but not limited to CPR and de-escalation.
• Attends and participates in rounds and case conferences when appropriate.
• Maintains the physical ability to participate in patient activities.
• Supports residents with daily routines, medical reminders, meal preparation, transportation, and personal care as needed.

Requirements

• High School Diploma or Equivalent required; Bachelor's Degree in a related field preferred.
• Medication Administration Program Certified (MAP - Massachusetts) - Massachusetts Department of Public Health required.
• Class D Passenger Vehicle Driver's License preferred.
• One-year patient care or related experience preferred, preferably in a psychiatric setting (1-2 years preferred).
• All employees must possess basic computer skills to use a variety of electronic or online systems (PRISM) for communication, clinical, and administrative purposes. Familiarity with Epic preferred.
• Ability to drive, including transporting patients, may be required based on role and location.

Pay

Pay Range: $23.40 - $30.73/Hourly (Grade MCL051). At Mass General Brigham, base pay is determined by skills, relevant experience, education, certifications, and other essential factors.
