Job Summary

Under direction of the Program Manager, the Community Residence Counselor (CRC) provides direct patient care and participates in therapeutic milieu within the scope of defined practice.

Essential Functions

  • Provides direct patient care in accordance with current treatment plan, under the supervision of the Nurse Manager, Clinical Coordinator and Charge Nurse.
  • Provides and supervises community activities according to needs.
  • Remains awake and alert, at all times, while on the job.
  • Aids patients in maintaining and keeping various appointments.
  • Clear, concise, and timely documentation and shift notes.
  • Establishes relationships with patients that are helpful and maintains professional boundaries.
  • Participates in patient rounds, status checks, or teams per program guidelines.
  • Completes assigned patient care tasks per program guidelines and/or daily assignment sheet.
  • Works effectively to manage the therapeutic milieu.
  • Provides patients with direction and supervision as needed.
  • Follows established hospital & program policies, procedures, and protocols.
  • Communicates information regarding patient milieu/therapeutic environment to appropriate staff in a timely manner.
  • Communicates observations related to medication side effects, physical and emotional changes to appropriate medical staff in a timely manner.
  • Encourages patients in their activities of daily living.
  • Affirms sleep patterns and assists patients to develop more appropriate sleep cycles notifying clinical team as necessary.
  • Completes mandatory annual in-service requirements, including but not limited to: CPR and de-escalation.
  • Attends and participates in rounds and case conferences when appropriate.
  • Supports residents with daily routines, medical reminders, meal preparation, transportation, and personal care as needed.

Qualifications

  • Education: High School Diploma or Equivalent required and Bachelor's Degree in a related field of study preferred.
  • Licenses and Credentials: Medication Administration Program Certified [MAP - Massachusetts] - Massachusetts Department of Public Health required. Class D Passenger Vehicle Driver's License [State License] - Generic - HR Only preferred.
  • Experience: One-year patient care or related experience preferred, preferably in a psychiatric setting. 1-2 years preferred.
