About the Role

As the Community Outreach Worker, you will provide a wide variety of services to engage, support, advocate, and educate community members in overall health and wellness. You will serve as a liaison between health and social services in the community to facilitate access and improve health outcomes by increasing health knowledge and self-sufficiency. This includes connecting agencies to provide short-term care coordination and linking clients to resources and support programs to improve their health and general well-being through education, coordination of care, and services.

This unique role works in both clinical and community-based settings, including clients' homes.

Responsibilities

  • Engage, support, and advocate for community members to enhance health and wellness.
  • Serve as a liaison between health and social services to improve access and health outcomes.
  • Provide short-term care coordination and connect clients to resources and support programs.
  • Educate clients to improve health knowledge and self-sufficiency.
  • Work in clinical, community-based settings, and clients' homes.

Requirements

  • High School diploma or equivalent.
  • Current American Heart Association Health Care Provider Basic Life Support (BLS) CPR card.
  • Current driver’s license and valid Hawai‘i auto insurance with short notice access to an automobile for site visits.
  • One (1) year of experience in customer service or a related field.

Preferred Qualifications

  • Experience in population health management, community outreach, community assessment, or advocacy.

Skills

Pay

Pay Range: 20.18 - 25.23 USD per hour. Rate of pay for selected candidates will be determined by various factors including knowledge, skills, abilities, relevant experience and training, as well as internal peer equity.
