The referenced base salary range represents the low and high end of University of Maryland’s Faculty Physician’s Inc. salary range for this position. Some candidates will not be eligible for the upper end of the salary range. Exact salary will ultimately depend on multiple factors, which may include the successful candidate’s geographical location, skills, work experience, market conditions, internal equity, responsibility factor and span of control, education/ training and other qualifications.

About the role

Under direct supervision, visits individuals and families in their homes, in shelters or other similar places to provide basic community outreach services and to assist with social interventions necessary in coping with a new diagnosis.

Responsibilities

  • Visits individuals and families in their homes, in shelters or other similar places to provide basic community outreach services and to assist with social interventions necessary in coping with a new diagnosis.
  • Provides support and resources to individuals and families dealing with a new diagnosis.
  • Assists with identifying and addressing social determinants of health.
  • Collaborates with healthcare providers and community partners to enhance care coordination.

Requirements

  • High School Diploma or equivalent (GED)
  • Certification as Community Health Worker from an accredited training program
  • Minimum 2 years of experience as a CHW or outreach worker with a broad background with diverse patient populations, preferably in a medical setting

Qualifications

  • Broad background with diverse patient populations
  • Experience in a medical setting preferred

Skills

  • Excellent communication and interpersonal skills
  • Ability to build rapport and trust with clients
  • Strong organizational and time management skills
  • Knowledge of community resources and services
