Community Outreach Driver - Mobile & Street Medicine

Key Responsibilities

• Transportation & Vehicle Maintenance:
  • Retrieve and deliver the mobile medical van.
  • Prepare the van for daily operations.
  • Safely transport team members and clients.
  • Maintain vehicle cleanliness and appearance.
  • Coordinate vehicle maintenance and repairs.
• Outreach & Engagement:
  • Conduct outreach to identify and connect with individuals experiencing homelessness.
  • Provide health education and information.
  • Advocate for client needs and coordinate care.
  • Implement health promotion activities.
  • Treat clients with respect and dignity.
  • Offer substance use counseling and support.
  • Support mental health needs.
  • Implement harm reduction practices.
• Security & Team Support:
  • Work collaboratively with the multidisciplinary team.
  • Ensure safe passage and provide transportation support.
  • Maintain vigilance for team safety.
  • Utilize de-escalation techniques.
  • Respond to medical emergencies.
• Administrative:
  • Maintain vehicle logs and documentation.
  • Document care in the EMR.
  • Track patient outreach attempts and visits.
  • Organize outreach items and supplies.
  • Attend and participate in team meetings.

Qualifications

• High school diploma or equivalent.
• BLS or CPR & first aid certification.
• Valid California Class C driver’s license.
• At least 2 years of experience in community health, outreach, or a related field.
• Experience driving large vehicles.
• A valid California Class A or B driver's license.
• Bilingual in Spanish.
• AOD registration or certification.
• Peer Specialist Certification or Substance Use Disorder Counselor certification.
• Professional driving experience.
• Experience providing counseling and support to individuals with substance use disorders.
• Personal lived experience with homelessness, substance use, or involvement in the justice system (preferred).

Physical Requirements

• Sitting: 3 hours.
• Standing: 2 hours.
• Walking: 3 hours.
• Driving: Frequently.
