About the role
The Community Outreach Coordinator will support communities throughout the Inland Empire by planning, coordinating, and executing a variety of outreach initiatives.
Responsibilities
- Plan, coordinate, and perform various outreach activities including site visits to build awareness of organization's services.
- Represent the organization and/or the program at community meetings and events.
- Distribute monthly calendars, newsletters, and outreach publications.
- Interface with community and education leaders to distribute program/organization materials.
- Generate reports including program data, dashboards, outreach activities, and community contracts.
- Collaborate with community agencies to develop and collaborate in new or existing partnerships.
- (If conducting outreach activities for mobile clinics): Conduct patient registration, enter relevant patient records into electronic health records, schedule appointments, and follow-ups.
- Participate in the development and implementation of patient/family health education.
Requirements
- High School/GED required.
- At least 2 years experience participating in community outreach activities.
