About the role
The Community Outreach Assistant (II) will play a vital role by assisting the Wellness Manager of the Petersen HIV Clinics to help vulnerable patients to link to needed community resources. The position is part-time and may end on May 31, 2027. The University of Arizona offers innovative work-life programs.
Responsibilities
- Assist patients in accessing community resources by identifying appropriate services and providing necessary referrals.
- Ensure patients receive the support they need by connecting them with relevant external programs and services.
- Participate in care planning and follow-up activities through in-person, virtual, and phone interactions with patients.
- Provide administrative support during these sessions by assisting with scheduling, documentation, and coordination efforts to facilitate effective care and resource utilization.
- Collect and monitor behavioral health referrals to improve retention and viral load suppression rates for patients reporting behavioral health conditions.
- Afford outreach to current and new resources in the southern Arizona communities to forge partnerships with behavioral health agencies to promote health equity and care coordination for patients.
- Organize partner meetings to review partnership needs and service updates, such as meet and greet, phone call, or virtual meeting.
- Aid in the pursuit of formal agreements with collaborations, such as letters of agreement or MOUs.
- Maintain a resource guide for Petersen HIV Clinics staff and patients that identifies behavioral health support services and other resources within the clinic's service area.
Requirements
- High School diploma or equivalent is required.
- Three (3) years of relevant experience, or equivalent combination of education and work experience is required.
Qualifications
- Current ASU MSW student preferred.
Skills
- Excellent communication skills.
- Ability to build and maintain relationships with community partners.
- Proficiency in data management and resource organization.
