Community Medical Director -Mostly Remote- Rochester, NY

This role supports clinical priorities focused on quality, patient-centered outcomes, and effective care delivery. You will collaborate with clinical leaders, operational teams, and cross-functional stakeholders to improve standards, workflows, and execution within a healthcare environment.

Location: Remote – US-based candidates only, no visa sponsorship available.

Responsibilities

• Oversee and guide the clinical Community Team
• Collaborate with interdisciplinary clinicians for patient care
• Manage relationships with Market GM and Senior Medical Officer
• Analyze local and national health policy impacts
• Identify opportunities to improve community healthcare outcomes
• Build relationships with external healthcare facilities
• Float within the community physician role as needed

Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required
• Current and valid state medical license
• 5+ years managing an interdisciplinary team
• 5+ years community physician experience in geriatric care
• 3+ years managing P&L responsibilities
• Geriatric and palliative care certifications preferred
• Basic Life Support (BLS) certification required

Pay

$193,000 – $387,000 annually
