Community Medical Director -Mostly Remote- Chicago, IL

This role supports clinical priorities focused on quality, patient-centered outcomes, and effective care delivery. You will collaborate with clinical leaders, operational teams, and cross-functional stakeholders to improve standards, workflows, and execution, strengthening care quality, team performance, and measurable outcomes within a healthcare environment.

Location: Remote - US based candidates only, no visa sponsorship available.

Responsibilities

• Oversee the clinical Community Team to manage patient health and wellbeing
• Collaborate with various clinicians and operational teams
• Build relationships with hospitals and SNFs for improved patient outcomes
• Develop strategies to enhance community performance metrics
• Foster key relationships within the Community Network
• Float within community physician role as needed
• Partner with the company Leadership to drive organizational growth

Qualifications

• MD or DO degree
• Current and valid state medical license
• 5+ years managing an interdisciplinary team
• 5+ years as a community physician providing geriatric care
• 3+ years managing a P&L
• Geriatric certification preferred
• Palliative care certification preferred
• BLS certification required
• Strong computer skills, including experience with EHRs

Pay

$193,000 – $387,000 annually
