Community Life Coach

About the role

Under the supervision of the Manager, the Community Life Coach (CLC) helps people with intellectual disabilities connect to their communities through various activities and opportunities.

Responsibilities

• Identify community resources and activities relevant to participants' interests and skill levels.
• Facilitate participation in typical and integrated activities, events, and organizations in the participant's neighborhood or local community.
• Foster connections between participants and persons without disabilities to enhance social capital.
• Accompany participants to activities, if needed, and facilitate their involvement.
• Communicate progress, successes, and challenges to guardians/care providers as needed.
• Ensure personal safety of participants on the caseload.
• Provide support to maintain or enhance personal independence.
• Provide attendant care and/or other supports as necessary (e.g., personal care, medication assistance).
• Support AbleLight's Mission, vision, and Core Values while empowering each person to direct their own life choices.
• Perform other duties as assigned.

Requirements

• High school diploma or G.E.D. required.
• Must be 18 years of age or older.
• Knowledgeable in Microsoft Office and capable of using electronic charting programs.
• Valid drivers' license with a good driving record as defined by AbleLight policy.
• Successfully complete Community Life Coach training.
• Clear background check as defined by regulations and policies.
• Able to lift at least 50 pounds.
• Able to stand for 8 hours at a time.
• Able to establish and maintain professional and cooperative relations with local businesses and agencies.
