Community Liaison Nurse

Responsibilities

• Build strong relationships with healthcare teams, patients, families, and community partners.
• Coordinate transitions of care to ensure seamless patient experiences.
• Provide education on available services and community resources.
• Promote healthcare programs within the community.
• Collaborate with physicians, care managers, and external agencies to enhance access, reduce readmissions, and support wellness initiatives.
