Community Nurse Liaison RN
About the role
The Clinical Nurse Liaison provides patient evaluations, collaborates with hospital personnel to determine patient’s clinical needs, and assesses appropriateness for admission to a skilled nursing facility. This position works closely with communities and healthcare partners and also verifies benefits and payor sources.
Requirements
- Minimum of three years of clinical experience in an acute care or long-term care setting
- Current Indiana RN License
