Community Liaison for Hospice

Full time – 40 hours a week, Monday through Friday from 8 am to 4:30 pm, with some weekend coverage supporting Missouri BJC Hospice service areas. Mileage reimbursement at the IRS standard rate.

About the Role

BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment, and high-tech respiratory care. Specialty home care programs are also available, including adult and pediatric asthma, cardiac, diabetes, orthopedic, and wound care programs. BJC Home Care provides care to thousands of patients in both Missouri and Illinois, serving more than 25 counties. It has become the largest home care network in the region and one of the largest in the country.

The Marketing team of BJC Home Care Services works to promote the organization's services and facilitate patient referrals from physicians, hospitals, and other healthcare facilities in the geographic areas where we provide care.

Responsibilities

  • Advocate for the Medicare benefit and educate on services available from BJC Hospice, Home Infusion, Home Health, and/or Home Medical Equipment to members of the healthcare and patient community.
  • Develop community relationships and maintain positive relationships with customers and referral sources.
  • Respond to customer and community needs and describe the Medicare Conditions of Participation to increase awareness of available services and program requirements.
  • Build and maintain effective relationships with key referral sources.
  • Develop, facilitate, and coordinate the referral process to promote a seamless, timely, and comfortable transition experience for referring providers, patients, and their families.
  • Provide presentations of hospice philosophy and program requirements to an array of healthcare and community audiences.
  • Identify areas to increase referrals and develop and implement marketing plans.
  • Serve as a liaison between BJC Home Care Services and clients, actively participating and engaging in community partnerships.

Requirements

  • Bachelor’s Degree (required).
  • 2–5 years of experience (minimum); 5–10 years preferred.
  • No supervisory experience required.
  • Valid Driver’s License and Automobile Liability Insurance.
  • Preferred qualification: RN licensure.
