Community Liaison
About the role
The Community Liaison is responsible for creating and maintaining positive customer relationships to increase community awareness and referrals for Home Health, Hospice, Home Infusion, PACE, Transitional Care Unit, and other services within assigned accounts. This role includes working with discharge planners, facility staff, physicians, and individuals to assess appropriateness for hospice and home health services, facilitate coordination of services, and ensure appropriate documentation for compliance with agency and statutory guidelines to manage the transition of the patient from an acute care facility to the patient's home or next location of residence.
Responsibilities
- Build and maintain positive customer relationships to drive community awareness and referrals.
- Work with discharge planners, facility staff, physicians, and individuals to assess patient needs for hospice and home health services.
- Facilitate coordination of services and ensure compliance with agency and statutory documentation guidelines.
- Manage the transition of patients from acute care facilities to home or next residence.
Requirements
- Bachelor's Degree in Business, Healthcare Administration, Marketing, or a clinical discipline. OR Associate's degree plus four years of work experience; OR high school diploma plus six years of work experience (equivalent to a Bachelor's degree).
- 1 year of successful medical sales account management experience.
Preferred Qualifications
- Coursework or training focused on healthcare.
- 1 year of sales experience in home health, hospice, or infusion marketplace.
- 1 year of experience selling directly to physicians, hospitals, or discharge planners.
- Experience with customer relationship and territory management software.

Pay
The pay range for this position is $33.63 - $52.22 per hour (base pay). Additional compensation may be available, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
Schedule
Full time, Day shift.
