Community Liaison - Adult Day Health

About the role

YourADH is a mission-driven healthcare organization dedicated to enhancing the health, independence, and quality of life of older adults and individuals with disabilities. The Community Liaison supports the growth and success of our Adult Day Health (ADH) program by building and maintaining referral relationships, guiding prospective participants and families through the admissions and enrollment process, and connecting individuals and families with programs that enhance independence, health, socialization, and overall quality of life.

Responsibilities

  • Develop and maintain strong referral relationships with hospitals, physician offices, skilled nursing facilities, social workers, case managers, elder service agencies, senior housing communities, and other community partners.
  • Conduct regular in-person outreach visits to referral sources throughout the service area.
  • Represent the organization at community events, health fairs, networking opportunities, educational programs, and outreach initiatives.
  • Deliver presentations, in-services, and educational sessions to healthcare providers, community organizations, and stakeholders.
  • Identify opportunities to expand referral networks and strengthen the organization's market presence.
  • Manage the full intake and admissions process from initial referral through participant enrollment and admission.
  • Serve as the primary point of contact for prospective participants, caregivers, and referral sources throughout the enrollment process.
  • Coordinate intake activities including inquiry follow-up, scheduling assessments, collecting required documentation, and facilitating enrollment completion.
  • Conduct facility tours for prospective participants, families, referral partners, and community stakeholders.
  • Collaborate closely with clinical and operational teams to ensure smooth onboarding and successful admissions.
  • Support brand awareness initiatives through outreach campaigns, community partnerships, presentations, and marketing efforts.
  • Aid in the development, organization, and distribution of marketing and promotional materials.
  • Plan and participate in open houses, educational events, and community engagement activities designed to increase program visibility.
  • Maintain an active referral pipeline and track outreach efforts, referrals, follow-up activities, and outcomes.
  • Meet admissions, census growth, and qualified referral goals.
  • Monitor referral trends, market conditions, and competitive activity and provide feedback to leadership.
  • Collaborate with leadership to enhance outreach strategies and strengthen community recognition.
  • Participate in team meetings and organizational growth initiatives.

Requirements

  • Bachelor's degree in Healthcare Administration, Human Services, Marketing, Business, Social Work, or a related field preferred.
  • Minimum of 1–3 years of experience in healthcare outreach, community relations, admissions, business development, marketing, or related fields.
  • Experience working with older adults, caregivers, adult day health programs, home care, rehabilitation, senior living, healthcare organizations, or community-based services preferred.
  • Existing relationships within the local healthcare, senior services, or community services network are highly desirable.
  • Strong interpersonal, communication, networking, and relationship-building skills.
  • Ability to work independently while managing multiple priorities and deadlines.
  • Excellent organizational skills with strong attention to detail.
  • Customer-service focused with the ability to build trust and rapport with participants, families, and referral partners.
  • Proficiency with Microsoft Office and CRM/referral tracking systems.
  • Valid driver's license and reliable transportation required.
  • Frequent local travel throughout the service area is required.
  • Ability to communicate effectively in person, by phone, and electronically.

Qualifications

  • Bachelor's degree in Healthcare Administration, Human Services, Marketing, Business, Social Work, or a related field preferred.
  • Equivalent combination of education and relevant experience will be considered.

Competitive Pay Rates

Schedule

Regular walking, standing, and sitting throughout the workday. Frequent local travel throughout the service area is required. Ability to conduct presentations, tours, and community outreach activities. Work environment includes a combination of office-based administrative work and community-based field outreach.
