Community Impact Specialist
About the role
Community Medical Services (CMS) is hiring a Community Impact Specialist to assist patients recovering from addictions to opioids and other illicit substances. Every day you will use your experience in substance abuse rehabilitation and opioid treatment programs to foster relationships with community partners.
Responsibilities
- Use your experience to support CMS clinic staff in providing a safe place for anyone looking to start their recovery journey with us.
- Coordinate care between opioid treatment program clients and community agencies.
- Access and provide community resources to clients.
- Document all client/agency encounters, coordination of care efforts, and community outreach activities, presentations, and meetings.
- Maintain outreach program data in multiple databases, including QuickBase, Methasoft, and CRM.
- Maintain appropriate coordination of care with all systems and provide documentation as requested from system partners.
- Support, including intake documentation, to special populations and projects.
- Resolve client altercations, manage crisis episodes, and assist in conflict resolution.
- Attend provider and community meetings for the purpose of advocating, educating, and supporting system partner relationships.
- Support program development and implementation with the project management team.
- Process referrals for community support from multiple CMS clinics and community system partners.
- Complete documentation within 24 hours of shift.
Requirements
- Experience in substance abuse rehabilitation and/or opioid treatment programs (preferred).
- Bilingual (preferred).
- Ability to travel within the region, including to all CMS locations as needed.
- Culturally competent and sensitive to individualized client treatment needs.
- Valid Driver License and Motor Vehicle Clearance (satisfactory driving record for 39 months).
