About the role

ReGenesis Health Care is seeking a full-time Community Health Worker to support patients and their families in accessing community services and adopting healthy behaviors.

Responsibilities

• Connecting with the community
• Coordinating community events and health education materials
• Advocating for community needs and resources by attending meetings and events
• Managing a caseload of patients
• Providing health education
• Maintaining patient records and documenting using computer systems
• Helping patients feel comfortable discussing their social drivers of health
• Interacting positively with a diverse patient population

Requirements

• High School Diploma or equivalent education
• Post-secondary degree in related studies (Public Health, social work, psychology, or related human services field) preferred
• Community Health Worker Certified preferred

Qualifications

• Experience connecting with the community
• Experience coordinating community events and health education materials
• Experience advocating for community needs and resources
• Experience managing a caseload of patients
• Experience providing health education
• Ability to read, write, and maintain patient records and documents using computer systems
• Ability to help patients feel comfortable discussing their social drivers of health
• Ability to interact positively with a diverse patient population
