About the role

The Community Health Worker/Patient Navigator (CHW) will support our Transitions of Care Team by conducting outreach, identifying and recruiting patients, and helping them engage in care and care management services. The CHW will provide patient education and advocacy to assist patients with accessing services at FHCW and making and following up on referrals to external services as part of the population health team. This role supports both the Care Management Team and the Population Health Team.

Responsibilities

  • Supports the care team with patient follow-up, reminders, and addressing barriers to care.
  • Documents all services as required and assists in meeting all program requirements.
  • Collects and analyzes data related to the project and works with data management staff for data collection.
  • Provides outreach and meets with patients to explain services and coordinates the treatment plan.
  • Facilitates the referral process for patients using Healthify, E-referral, Community Help, and other applicable tools.
  • Advocates for patients regarding transportation, interpretation, health insurance, and FHCW services.
  • Acts as a hub of information across multiple departments with strong organizational and communication skills.
  • Performs any and all other duties as assigned.

Qualifications

  • High school diploma or GED required.
  • Bachelor’s degree is preferred.
  • Preferred candidate will have a Community Health Worker or Medical Interpreter certificate of completion.
  • Minimum of two years of relevant work or college-level internship experience required.
  • Proven ability to perform patient interviews, coordinate and collaborate with internal services and external community partners.
  • Bilingual in English and another language appropriate to FHCW patient population required.
  • Community Health Worker Core Training required within the first year of employment.

This position reports to the Director of Population Health.
