About the role
A Community Health Worker (CHW) is a front-line public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the worker to serve as a liaison/link between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. Performs duties and responsibilities in a manner consistent with Mercy’s mission, values, and Mercy Service Standards.
Requirements
- High School Diploma or equivalent
- 1-3 years CHW experience or equivalent
- Mercy-provided Community Health Worker training certificate within 6-12 months
- Basic Life Support certification
- Strong relationship and collaboration skills
- Excellent communication skills
- Ability to work independently
- Compassion for poor and underserved members of the community
- Proficiency with Microsoft Applications and ability to document in an EMR
- Access to reliable transportation

- Additional language skills applicable to the community served
