Community Health Worker (CHW) - Mobile Clinic - Full Time - Days - 8hr West Covina

About the role

The Community Health Worker (CHW) is a frontline member of the mobile clinic team, responsible for patient engagement, outreach, navigation of social services, and supporting clinical staff in delivering high‑quality care. This role helps bridge gaps in access by meeting patients where they are—streets, shelters, encampments, schools, and community centers—while building trusting, culturally responsive relationships.

Responsibilities

• Engage patients in diverse community settings, including streets, shelters, encampments, schools, and community centers.
• Conduct outreach to underserved or vulnerable populations.
• Navigate patients to appropriate social services and community resources.
• Support clinical staff in delivering care during mobile clinic visits.
• Build trusting, culturally responsive relationships with patients and families.
• Ensure patients receive the resources they need to improve health and stability.
• Travel to multiple sites and work in outdoor, mobile, and unpredictable environments.

Requirements

• Education
  • High school diploma or equivalent (required).
  • Additional education in health or social services (preferred).
  • Bilingual in Spanish (preferred).
• Experience
  • Experience working with underserved or vulnerable populations (e.g., homelessness, low‑income communities, immigrants).
  • Strong communication skills and ability to build trust with diverse groups.
  • Strong organizational skills and comfort with basic technology/data entry.
  • Knowledge of local community resources and social service systems (preferred).

  • None required.

Pay

$23.00 – $34.50 per hour
