About the role
Coordinates outreach, education, and care coordination services to engage patients in preventive and ongoing healthcare, support closure of care gaps, improve clinical outcomes, and promote effective use of healthcare resources. Assists patients in addressing clinical and non-clinical needs by facilitating access to community resources, supporting treatment adherence and self-management, and helping remove barriers to achieving care plan goals.
Requirements
- High School diploma or GED
- BLS certification
Preferred qualifications
- Community Health Worker certification
- Previous CHW or care coordination work with community resources
Schedule
Full Time (80 hours per 2 week pay period), Monday–Friday 8:00 am–4:30 pm
