About the role

Supports community health initiatives by providing direct patient care, education, and outreach services. Works closely with healthcare providers, community members, and other stakeholders to improve health outcomes.

Responsibilities

• Provide direct patient care and education services

Requirements

• Bachelor's degree in public health, healthcare administration, or related field
• Experience in community health or related field
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team

Qualifications

• Current CPR certification
• Valid driver’s license and reliable transportation
• Passing score on a background check
