About the role
Supports community health initiatives by providing direct patient care, education, and outreach services. Works closely with healthcare providers, community members, and other stakeholders to improve health outcomes.
Responsibilities
- Provide direct patient care and education services
- Conduct health screenings and assessments
- Develop and implement health promotion programs
- Collaborate with healthcare providers and community partners
- Document patient interactions and progress
Requirements
- Bachelor's degree in public health, healthcare administration, or related field
- Experience in community health or related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
Qualifications
- Current CPR certification
- Valid driver’s license and reliable transportation
- Passing score on a background check
