About the Role
Sentara Health is seeking a Community Health Worker to support community outreach, resource coordination, and care navigation for individuals in the community. This role works in collaboration with the Case Manager and other healthcare team members to provide comprehensive and coordinated care, engaging in interventions to improve self-management skills and quality of life for patients with chronic and/or complex illnesses.
As a patient advocate and healthcare navigator, you will assist with resource allocation and management, bridging the gap between healthcare delivery systems and community health services. Responsibilities include increasing culturally relevant knowledge about health and wellbeing, improving access to services for underserved populations, connecting patients to community resources, and ultimately improving health outcomes.
Schedule
- Full-time, 40 hours per week
- Monday–Friday, 8:00 AM–6:00 PM (hours vary)
- Occasional weekend coverage for events (Saturday–Sunday, 8:00 AM–6:00 PM; hours vary)
- Position will never exceed more than 40 hours a week
- Mileage reimbursement provided for event-related travel
Requirements
- High school diploma or equivalent Required
- Valid driver’s license Required
- Knowledge of medical terminology preferred
- Community Health Worker (CHW) certification preferred
Experience
- Experience working in a community-based setting preferred
- Experience identifying and coordinating community resources preferred
- Epic experience is a plus
- Spanish-speaking ability strongly preferred
