Community Health Specialist II

About the role
The Community Health Specialist II delivers eight annual virtual state-approved foundational Community Health Worker (CHW) training cohorts, managing scheduling, instruction, and learner support for each cohort cycle. This position also provides Mental Health First Aid (MHFA) training to CHW trainees following MHFA trainer certification. Close communication with grant leadership, trainees, and state leadership is required. The role leads regular training and implements the grant work plan to remain compliant with funder expectations. This position is grant-funded; continued employment is contingent upon continuation of those funds.
Responsibilities
- Manage, plan, and coordinate eight annual virtual state-approved foundational CHW training sessions and MHFA training sessions.
- Lead and manage applicable partnerships; develop and implement grant-related activities to work toward grant goals.
- Update Trager executive staff on grant progress and meet required grant reporting deadlines.
- Facilitate the structured post-training mentorship program for all trainees.
- Occasional travel and weekend work may be required.
Requirements
- Bachelor’s degree in a related field and two years of relevant experience or an equivalent combination of education and experience.
- Expert-level experience and comfort with technology, including troubleshooting remote training issues.
Preferred Qualifications
- Previous work experience on grants, in community health work, and with local community resources and services.
- Experience in healthcare settings, including evaluation, grant implementation, and community health applications.
- Experience facilitating training for learners with varying levels of experience.
- Certified as a Community Health Worker in Kentucky, with certification in good standing.
Pay
Target compensation range: $48,225 – $72,389, commensurate with candidate experience.
