Community Health Worker II

About the role

Community Health Workers (CHW) may assist with a variety of concerns impacting individuals and families, including but not limited to the control and prevention of chronic conditions or infectious diseases, behavioral health conditions, and the need for preventive services. CHW services can help patients receive appropriate services related to perinatal care, preventive care, sexual and reproductive health, environmental and climate‑sensitive health issues, oral health, aging, injury, and domestic‑violence prevention. CHWs are typically members of the communities they serve and play a key role in linking health and social services for patients. They provide culturally relevant connections to healthcare and address Social Determinants of Health (SDOH) alongside general and specialty medical services, drawing on their lived experience and that of their families and social circles.

Responsibilities

• Outreach and Engagement – conduct street, community, and online outreach to inform and educate community members and institutions about St. John’s and partner agency services, and engage individuals and families.
• Health Education – provide education on general and specific health conditions (e.g., diabetes, heart disease, pulmonary disease, hypertension, HIV/AIDS, hepatitis, STIs, substance use disorders, mental illness) and on how addressing SDOH improves overall well‑being.
• Screening and Linkage to Care – perform brief screenings to determine service needs and provide warm‑handed referrals to culturally relevant services within St. John’s and the community.
• Assessment and Service Planning – use approved tools to assess SDOH, create individual and family service plans, and document needs, action steps, and progress; employ clinically intensive tools such as PHQ‑9, GAIN‑SS, Columbia Depression Scale, Beck Depression and Anxiety Inventories.
• Case Management, Care Coordination, and System Navigation – assist individuals and families in identifying and accessing culturally competent services, ensure communication among medical and psychosocial providers, and follow up on referrals.
• Peer Support, Education, and Advocacy – provide lay counseling, appointment reminders, transportation assistance, cultural mediation, and teach navigation of St. John’s and community service networks.
• Training – develop and deliver trainings on health conditions and SDOH to St. John’s staff, community partners, and broader medical and social service systems.
• Provide a higher level of education to paraprofessional staff in various disciplines.

Qualifications/Licensure

• Education: Preferred – Bachelor’s degree or equivalent experience.
• Experience: Required – Two years of relevant experience.
• Bilingual (preferred).
