Community Health Worker II

Schedule: Monday - Friday, 7:30am - 4:00pm. Purpose Statement/Position Summary: The Community Health Worker II, under general supervision of department leadership, provides specific services to patients and their families. Coordinates case management and social care services. Demonstrates commitment to the principles of family-centered care. Serves as the first level of support to community health worker team members for resource and training questions. Minimum Qualifications/Work Experience Required: 2 or more years of experience as a community health worker, parent partner, navigator, case manager, or similar position. 1 or more years of experience with community resource navigation, social-care support, and/or health education. Preferred: Experience training team members and facilitating presentations, preferred. Experience with public/government benefits through work or lived experiences, preferred. Bilingual skills, preferred. Education/Licensure/Certification Required: High school diploma/GED OR 4 years of related education and/or work experience.
