Job Summary
Perform direct outreach to target population as listed on program grant and/or as directed by supervisor.
Essential Job Functions
- Provide basic information on health topics as directed by Supervisor
- Conduct comprehensive outreach (and/or in-reach) in local area, and refer individuals to program grant activities and/or clinic
- Complete appropriate paperwork
- Conduct basic assessments/screenings of target population
- Complete data collection forms
- Participate in health fairs and other community events
- Track incentive cards
- Demonstrate familiarity with VCC programs and services
- Meet minimal requirements of program objectives
- Database entry
- Attend meetings, as needed
- Enhance professional growth and development through participation in educational programs, current literature review, in-service meetings and workshops
- Support the vision, mission and goals, and demonstrate a commitment to the values, of the organization
Minimum Qualifications
- High school graduate or equivalent
- Minimum two years' experience in community education, child development, health education and/or community engagement
- Minimum two years' driving experience with valid California driver’s license and current vehicle insurance
Preferred Qualifications
- Bilingual English/Spanish
- Experience/familiarity with computers, Microsoft Office products (Word/Excel), business e-mail communication systems and internet search capabilities
Skills/Knowledge/Abilities
- Demonstrated ability to communicate effectively verbally with target population and all levels personnel staff in an appropriate manner
- Ability to work independently, take initiative, and be proactive
- Experience/familiarity with computers, Microsoft Office products (Word/Excel), business e-mail communication systems and internet search capabilities

Pay Range
$21.00 - $23.00 Depending On Experience
