About the role

The Community Health Representative promotes Family HealthCare Network services through outreach activities, coordination of community events, and participation in relevant grant initiatives and special programs.

Responsibilities

  • Carry out responsibilities in community outreach, enrollment assistance, coordination of FHCN community events, grants, and programs.
  • Achieve departmental goals and metrics for assigned functional areas.
  • Communicate feedback to the supervisor regarding workflows and policies.
  • Serve as a subject matter expert in assigned functional areas.
  • Perform high-level administrative functions to comply with regulations and reporting requirements to meet department deadlines.
  • Execute outreach activities for the organization.
  • Participate in community events to disseminate information on FHCN services and programs and provide health screenings.
  • Participate in relevant community meetings and committees as a network representative.
  • Lead community focus groups and surveys for the network.
  • Execute network-hosted community events.
  • Conduct presentations and educational workshops to raise awareness of network services and programs.
  • Maintain relationships with strategic partners and participate in mutually beneficial activities.
  • Act as a liaison between FHCN and patients.
  • Support consumers with enrollment services and referrals.
  • Conduct intake and screenings to determine eligibility for Medi-Cal, Medicare, Covered CA, CalFresh, sliding fee, food pantries, and literacy programs.
  • Educate consumers on health coverage benefits and rights and assist with navigating services.
  • Provide ongoing case management to ensure healthcare coverage retention and utilization of FHCN services.
  • Complete and maintain all obligatory certifications as required.
  • Stay abreast of State and local program updates.
  • Execute FHCN programs and departmental grant initiatives.
  • Implement programs and grant curriculums at sites and in community settings.
  • Execute day-to-day operations at food pantries and record daily inventory of food items and related materials.
  • Receive product deliveries and ensure items are refrigerated and stored appropriately.
  • Participate in events to meet program and grant objectives.
  • Ensure compliance with all program or grant-related training and certifications.
  • Maintain a professional working relationship with all program partners and grant funders.
  • Adhere to the Attendance and Absenteeism Policy; regular attendance is considered an essential function of the position.
  • Ability to present to and work at any FHCN location, both at the beginning of a shift or during a shift, based on business need.
  • Perform other duties as assigned.

Requirements

  • Effective June 1, 2026, all individuals hired into the role must possess work-related skills at a higher level than completion of high school, including formal written and verbal communications skills, computational and computer skills, mathematical, and technical skills.
  • Proof of completion of high school with a minimum overall GPA of 2.5 or completion of General Educational Development (GED) with a minimum overall score of 162.5. If an individual has completed a degree at a higher level with a stronger GPA, they may provide proof of GPA from that degree in lieu of the high school diploma.
  • Minimum of two years of experience in community outreach, educational activities, and customer service.
  • Valid CA driver’s license and car insurance.
  • As a condition of employment, all employees assigned to provide Covered California education and enrollments must obtain certification as a Certified Enrollment Counselor, including completing state-mandated training, passing state certification exams, and passing a Federal Background Check.
  • Any staff assigned to support food programs must obtain a food handlers certification upon hire.

Skills

  • Ability to prepare complex documents in Microsoft Word, including creating tables, charts, graphs, and other elements.
  • Ability to use Microsoft Excel to review and compile data, including the use of formulas, functions, lookup tables, and other standard spreadsheet elements.
  • Ability to create basic presentations in Microsoft PowerPoint.
  • Effective written communication skills, including compiling information in correspondence, reports, articles, or other documentation.
  • Ability to convey technical information to non-technical audiences.

  • Regularly required to sit and use repetitive hand movements to type and grasp.
  • Frequently required to stand or walk.
  • Occasionally required to stoop, reach above and below shoulder height, squat, climb, lift, and/or move up to 30 pounds.
  • Must work in environmental elements, including cold and hot weather.
  • Must travel from site to site and to other non-FHCN locations.

Pay

Min Hourly Rate: $21.56
Max Hourly Rate: $30.86
