Community Health-Referral RN

Duties & Responsibilities

• Travel within the city of St. Louis for this role.
• Mileage reimbursement paid out***
• Community Patient Care: Assess new patients’ needs and chart incoming clients into different living sites coordinated through Peter & Paul Community Services.
• Be responsible for medication management and vital sign monitoring
• Screen, communicate, and respond to patient needs appropriately.
• Administer ordered medications via various routes.
• Ensure patient privacy and comfort during the examination.
• Educate patients about health matters as needed.
• Ensure timely and accurate entry of medical information into EMR.
• Adhere to ethical nursing standards, policies, and procedures.
• Communicate to nurse practitioners and physicians on required medical information.
• Serve as a primary contact for NOVUS outside medical referrals and prior authorization services.
• Support the growth and advancement of Novus Health’s mission.

Required Qualifications

• RN license in the state of practice
• Two (2) years’ experience in Public Health, clinical care and/or work in infectious disease.
• Nonprofit and health clinic experience preferred.
• Previous knowledge and training working with STI and HIV services
• Advanced communication skills; verbal and written.
• Excellent customer service skills
• Flexibility to work with diverse patient populations
• Ability to work within scheduled clinic hours of operation
