About the role
We are seeking an experienced Registered Nurse (RN) to provide community-based care management services for individuals requiring coordinated healthcare support.
Responsibilities
- Coordinate patient care through an interdisciplinary care management approach.
- Develop, implement, and monitor individualized care plans.
- Manage a caseload of patients requiring ongoing care coordination.
- Conduct community-based and telephonic care management.
- Support continuity of care through utilization management, transfer coordination, and discharge planning.
- Cookordinate appropriate healthcare services and authorizations.
- Promote patient safety, quality of care, and positive health outcomes for high-risk populations.
- Collaborate with healthcare providers, caregivers, and community resources to ensure effective care delivery.
Requirements
- Required Education: High School Diploma or GED, Graduate of an accredited Registered Nursing program.
- Preferred Education: Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN).
- Required Certifications: Active Registered Nurse (RN) license, Current Basic Life Support (BLS) certification.
Qualifications
- Minimum of 2 years of experience in one or more of the following: Managed care, Case management, Utilization review, Discharge planning, Transitional care, Acute care, Critical care, Long-term acute care, Skilled nursing, Long-term care.
- Experience working with government-sponsored healthcare programs is preferred.
