Community Healing Unit Care Coordinator

Job Summary

The Care Coordinator provides lay counseling, case management, and systems navigation support to victims of crime, hate, and violence. They ensure clients receive holistic and wrap-around services through motivational interviewing, trauma-informed and culturally responsive care. The role involves coordinating with the Community Healing Unit (CHU) team, AHS departments, and external agencies to facilitate the integration of care and coordinating services.

Essential Job Functions

• Complete intakes on new referrals and assess for mental health, case management, systems navigation, and alternative healing needs
• Coordinate with Community Healing Unit staff and CHU Program Manager to develop care plan and to minimize gaps in services
• Communicate with specialty mental health providers, community-based agencies, and related government agencies for appropriate treatment, resources, services, and supports
• Administer lay counseling to clients (e.g. develop rapport, encourage healing, identify patient goals, provide emotional support, reflective listening)
• Provide case management with appropriate referrals to community resources
• Mentor mental health navigation, manage transition of care to external providers, and develop any needed transition plans
• Liaise with legal offices, law enforcement and government agencies, district attorney’s office, and other community-based organizations to provide systems navigation support (e.g. submit or follow-up with crime/incident reports, warm handoff or referrals to legal aid, link to victim resources)
• Connect victim to financial resources as needed (e.g. California Victims Compensation Fund)
• Follow up with clients via phone or other forms of communication with regards to services, referrals, and the client’s capacity to follow through with recommended care plans
• Troubleshoot care coordination, warm handoff, and referral efforts to ensure seamless continuation of care as needed
• Cover program phone line as backup when needed
• Lead department projects including but not limited to creating educational materials, planning cultural healing activities, organizing events, and making resource guides
• Conduct and document health education and outreach activities on various health topics (e.g., access to health care/health insurance, destigmatize mental illness, promote healing and alternative therapies to healing, etc.) in a linguistically and culturally appropriate manner with small-large groups and individuals
• Attend necessary staff meetings and trainings as designated by CHU Program Manager
• Develop and maintain contacts and working relations with staff at other organizations
• Aid with data collection and analysis for reports and grants under the supervision of CHU Program Manager
• Attend pertinent county and other grant-related meetings
• Represent AHS in various community forums, events, presentations, workshops, coalitions, and meetings as needed
• Provide interpreting or translation services during educational workshops and activities, special events, meetings, hearings, media events, educational materials, and other organizational activities as applicable
• Observe current codes of ethics and standards of practice for health care interpreting (e.g., interprets all speakers' messages accurately and completely; transparently carry out various interpreter interventions, such as cultural brokering, to facilitate clear communication between speakers; etc.)

General Agency/AHS Duties

• Foster an environment that promotes trust and cooperation among clients and staff
• Enforce clinic policies and procedures, including maintenance of client confidentiality under HIPAA privacy rules, to ensure that the principles of AHS are implemented
• Play an active role in planning organization-wide activities such as AHS general membership meetings, fundraisers, special clinic days, and other events
• Participate in general membership meetings, fundraisers, and other public events, as required
• Participate in outreach activities, and agency advocacy, and serve on ad hoc committees, as requested
• Attend AHS staff retreats and Board of Directors meetings, as required
• Participate in agency-wide quality improvement program processes and performance improvement teams to ensure excellence in the quality of services provided across the agency

Minimum Qualifications

• Bachelor’s degree in related field (i.e., social work, psychology, etc.) or equivalent experience
• Bachelor’s degree majors: health, social service, or other closely-related fields
• Experience working in the community health field
• Basic understanding and familiarity with concepts such as Motivational Interviewing, client-centered care, and trauma-informed care
• Ability to speak, read, and write at a high proficiency level in an Asian language (Cantonese, Mandarin, Vietnamese, Tagalog or Khmer strongly desired)

Preferred Qualifications

• Preferred bachelor’s degree majors: health, social service, or other closely-related fields
• Preferred experience working with patients and community members in a health care setting
• Preferred basic understanding and familiarity with concepts such as Motivational Interviewing, client-centered care, and trauma-informed care
• Preferred ability to speak, read, and write at a high proficiency level in an Asian language (Cantonese, Mandarin, Vietnamese, Tagalog or Khmer strongly desired)
