Responsibilities
- Provide services to individuals in their homes and communities according to their Habilitation Plans and individual Safeguards.
- Document Habilitation services contemporaneously in accordance with Agency, OPWDD, Medicaid and other requirements.
- Contribute to the development and implementation of the Staff Action Plan based on the person's Life Plan as developed by each individual, family, advocate, and Care Coordinator.
- Maintain the safety and welfare of individuals served, taking corrective actions as necessary and reporting concerns to the supervisor.
- Supervise individuals according to the Staff Action Plan and individual Safeguards.
- Build positive relationships and encourage advocacy for individuals' needs and rights.
- Encourage good personal health care and report health care concerns to supervisors or administrators.
- Complete required paperwork and documentation on a daily basis, including checklists, progress notes, injury/fall reports, seizure reports, attendance records, etc.
- Complete Monthly Summary Notes on time as required.
- Encourage community integration with an emphasis on inclusion, access, and safety.
- Travel in agency vehicles and follow transportation protocols, ensuring attendance checks are done each time individuals board and vacate the vehicle.
- Attend staff meetings and participate in supervision for professional growth and development.
- Complete all required initial and annual training as well as ongoing training to increase knowledge.
- Conduct fire drills, document results according to supervisor's instructions, and report incidents.
- Work overtime as needed to ensure proper staffing ratios.
- Adhere to and implement the NADSP Code of Ethics, Justice Center Code of Conduct, and NYS DSP Core Competencies.
- Follow Agency and Program Policies and Procedures, including HIPAA, Confidentiality, and the Corporate Compliance Plan.
- Other related duties as assigned.
Qualifications
- High School Diploma or GED.
- Ability to instruct others on vocational or daily living skills.
- Active listening and communication skills.
- Interpersonal and motivational skills.
- Analytical ability with problem-solving and follow-up.
- Independence and leadership skills.
- Proficiency in using a computer and appropriate software.
- Valid driver's license and ability to operate an agency vehicle if needed.
- Ability to speak English and, optionally, Bengali.
