Community Coordinator (S 30 hrs.)

About the Role

This is a full-time position (30 hours per week) with a schedule of Monday through Friday, 8:45 AM to 2:45 PM, located at Salem Day Services. The Community Coordinator will report to the Program Coordinator and is expected to uphold Triangle’s policies, foster a positive culture, and promote employment, self-advocacy, and independence.

Responsibilities

  • Supervises groups of participants in various settings, including work sites, volunteer sites, recreational outings, and other community outings.
  • Develops and maintains diverse volunteer, vocational, and educational opportunities for individuals.
  • Assists with the development of new and engaging curriculum to meet program goals.
  • Implements independent living, employment, self-advocacy, and soft skills into daily activities and curriculum.
  • Transports individuals to and from community activities and employment sites.
  • Prepares or assists in the preparation of required assessments and reports (e.g., incident reports, safety assessments, progress summaries).
  • Participates in identified training and professional development opportunities.
  • Intervenes in unsafe interactions or behavioral incidents to ensure individual safety.
  • Participates in ISP meetings with the Department of Developmental Services, participants, and their families or support teams as needed.
  • Collects accurate data as directed for ISPs, behavior plans, monthly progress notes, etc.
  • Collaborates with other Community Coordinators and the Program Coordinator to ensure quality service delivery.
  • Reports progress and issues to the Program Coordinator on a regular basis.
  • Positively represents Triangle to customers, family members, and community organizations.
  • Participates in biweekly team meetings and monthly supervision with the Program Coordinator.
  • Performs other duties as assigned.

Requirements

  • High school diploma or equivalent; some college in related coursework and/or one to three years of relatable experience preferred.
  • Massachusetts driver’s license and clean driving record.

Skills

  • Ability to verbally communicate, read, analyze, interpret, and write documents.
  • Ability to respond to and navigate inquiries or complaints from regulatory agencies or family members.

  • Requires close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
