Community Clinician (40 Hour) Office/On-Site #260623-0967FP-001

About the role

This position will play a pivotal role on the highest acuity, fidelity-model based team in outpatient services. This position will help to uphold the mission and vision of the agency, by being part of a team that does the majority of their work in the community, in order to meet clients where they are and to ensure the safety of the clients we serve and the communities and community members they live in proximity to. They will also work to reduce involvement with the criminal justice system and decrease frequent visits to the Emergency Departments (ED) and inpatient units, whenever possible.

Responsibilities

• Carry a full caseload, seeing clients primarily in the community;
• Work with clients to create person-centered treatment plans with shared goals and objectives;
• Help to keep their clients functioning as independently as possible while decreasing the need for frequent hospital/ED visits or involvement with the criminal justice system.

Requirements

• A Bachelor’s degree in a health care field and two (2) years of experience working independently using clinical case management techniques in a behavioral health setting.
• A Master’s degree in counseling, psychology, or other clinical discipline may be substituted for one (1) year of the General Experience.
• A current Connecticut license as a Registered Nurse OR hold a Multistate or Compact Registered Nurse license and two (2) years of experience working in a psychiatric setting may be substituted for the Bachelor’s degree.
• A Master’s degree in social work (MSW) and two (2) years at the level of a Mental Health Assistant 2 independently performing the full range of paraprofessional case management in a behavioral health setting may be substituted for the General Experience.
• Experience working in the community, experience working with individuals who have severe and persistent mental health and substance abuse needs
• Experience working on a high-paced, multidisciplinary team
• Experience providing direct support or case management services to adults with behavioral health and/or substance use disorders
• Experience performing intake assessments and person-centered recovery planning
• Experience completing clinical documentation

• Knowledge of behavioral health counseling techniques;

• social, cultural, economic, medical, psychological and legal issues which influence attitudes and behaviors of clients and families;

Pay

• $78,720* - $106,811/year (*New state employees start at the minimum)

Schedule

• Full-Time, 40 Hours per Week, First Shift, 8:30am - 5:00pm, Monday through Friday
