Community Benefits Enrollment Specialist

About the role

Family Health Centers of San Diego (FHCSD) has been providing caring, affordable, high-quality health care since 1966. Operating more than 90 sites across San Diego County, FHCSD serves over 227,000 patients annually, focusing on uninsured, low-income, and medically underserved individuals. Services include adult care, chronic disease management, pediatrics, comprehensive women’s care, dental, vision, case management, physical rehabilitation, speech therapy, infectious diseases, behavioral health, substance use counseling, and various specialty services.

Responsibilities

• Provide dedicated and specialized assistance for patients and clients.
• Work with FHCSD’s Enhanced Care Management, Community Support Team Members, and PACE to assist clients in applying for and filing claims/appellations.
• Gather medical records and other necessary information to complete applications.
• Write SOAR Medical Summary Reports for each individual’s application.
• Accompany individuals to set appointments at the Social Security Administration as needed.
• Coordinate visits to medical doctors, psychiatrists, and other healthcare specialists to gather evidence for the application process.
• Serve as a point of contact with public benefits entities on behalf of FHCSD patients and clients.
• Perform other duties as assigned.

Requirements

• A Bachelor’s degree in a related field preferred.
• Experience with health education or working in the medical/health or other social services related field.
• Two years of experience.
• Familiarity with Social Security Administration’s Listings of Impairments.
• Ability to prioritize tasks and meet frequent deadlines.
• Superior organizational skills and attention to detail.
• Good writing skills and the ability to analyze data and create reports.
• Strong verbal and written communication skills, including the ability to communicate with those who speak English as a second language.
• Computer experience, including troubleshooting and problem-solving, and proficiency in MS Office applications (Word, Excel).
