Community-Based Physician

About the role

The Physician for Primary Care, Adult and Pediatrics provides both well-care and acute/chronic care to patients across all ages, from infancy through adulthood, in a primary care setting. The physician is responsible for the comprehensive evaluation and documentation of the health status of patients, ensures high-quality care throughout their lifespan, collaborates with other healthcare professionals, educates patients and their families on health management, preventive care, and treatment plans, participates in case conferences, identifies barriers for patients and families, maintains agency medical records per agency policies, and consults with the Medical Director on complex cases.

Responsibilities

• Comprehensive evaluation and documentation of patient health status
• Collaboration with other healthcare professionals
• Educating patients and families on health management, preventive care, and treatment plans
• Participation in case conferences
• Identifying barriers for patients and families
• Maintaining agency medical records per agency policies
• Consulting with the Medical Director on complex cases

Requirements

• M.D. or D.O. degree
• A current, valid license to practice medicine in New York State (license must be in good standing)
• Maintain credentialing and privileges as granted by the Board of Directors and the Health Center
• Maintain DEA registration
• Not excluded to participate as a provider in the Medicaid and Medicare Programs
• Strong interpersonal and communication skills with compassion, and high level of service for patients, parents, and staff
• At least two years of experience providing medical care to patients and families
• Experience with supervision and/or training to clinical staff
