About the role
We are seeking a Clinical Nurse to support the General Internal Medicine (GIM) Faculty practice at Green Spring Station (GSS).
Responsibilities
- Care coordination for a group of complex patients to achieve clinical and organizational outcomes.
- Provide evidence-based care based on nursing process, policies, procedures, and protocols of the Johns Hopkins Hospital.
- Provide guidance to other team members to promote quality, safety, and excellence in care.
- Participate in Comprehensive Unit Based Safety Program (CUSP) initiatives.
- Perform transitional care management calls for practice patients discharged from hospitals in the area and determine the need for short-term outpatient follow-up.
- Provide direct patient care, when needed, for point of care testing, medication/vaccine administration, patient rooming and other duties as assigned.
- Provide patient education for self-care chronic disease management, such as, diabetes education (in person, by phone, or via MyChart).
- Collaborate with a multidisciplinary health care team to optimize team performance and patient outcomes.
- Traverse phone calls and electronic advice requests from patients and determine the need for medical assessment.
- Assess lab results and implement interventions in consultation with primary providers.
- Traverse calls from outside institutions, JHM Emergency Departments, and other health care providers; determine and implement follow-up plans.
- Identify care continuity needs of patients; generate and pend orders to primary providers for signature.
- Aid primary providers with maintaining electronic medical records; scan outside results into system, update immunization history, update health maintenance records, act on overdue health maintenance items.
- Convey critical action values/patient requests of lab results when requested by provider (communication from MD to RN).
- Help lead quality improvement initiatives that are focused on outcomes measures relevant to primary care.
- Aid Medical Office Coordinator (MOC) in scheduling patients for appointments.
- Help complete forms (e.g., disability, FMLA, mobility) requiring basic health history.
- Help obtain prior authorization for tests and medications.
- Provide oversight for ordering, stocking, and maintaining clinical supplies and equipment.
- Collaborate with clinic manager to ensure supplies, medications, and vaccines are available and up-to-date for clinical use.
- Responsible for other clinical duties as assigned.
Qualifications
- Individual must be a registered nurse, licensed in the State of Maryland.
- Maintain active licensure throughout employment.
- Experience with Epic preferred.
- Additional education may substitute for required experience, and additional related experience may substitute for required education beyond a high school diploma/graduation equivalent, to the extent permitted by the JHU equivalency formula.
- The individual must be a registered nurse, licensed in the State of Maryland or the state where practicing.
- Current American Heart Association or American Red Cross CPR Certification.
- Completion of the JHH Credentialing process.
