About the Role
This is a Hospice Certified Nursing Assistant role covering Bucks and Philadelphia Counties.
Responsibilities
- Administer patient care under the direction of an RN and as instructed by the CNA care plan.
- Report changes in the client’s condition and/or needs to the RN Case Manager.
- Provide personal care, including but not limited to bathing, shampooing, skin and nail care, oral hygiene, shaving, and dressing.
- Perform household tasks essential to healthcare, including meal preparation and feeding, laundry, and light housekeeping.
- Accurately document provided care and submit documents and pertinent observations following Hospice policy and procedures.
- Attend and/or complete in-service training to ensure compliance with the state requirement of 12 continuing education hours per year.
- Communicate effectively with all members of the interdisciplinary team.
- Monitor patient vital signs, including body temperature, heart rate, respiration rate, and blood pressure.
- Other duties as assigned.
Requirements
- High School Graduate (preferred).
- Current, valid Pennsylvania Nursing Aid license.
- Previous Hospice/Palliative experience.
