CNA/PCA - Home Services

Duties

• Provides personal care including (i.e. baths, back rubs, oral hygiene, shampoos, changing bed linen, assisting patients dressing and undressing, skin care to prevent breakdown, assisting the patient with toileting activities, light housekeeping, dressing changes, nail care, meal prep and assistance with feeding if necessary).
• Taking and recording oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered (with appropriate completed/demonstrated skills competency).
• Affixes and records vital signs.
• Affixes and records medications.
• Assists with transfers, ambulation and exercise according to the plan of care.
• Performs range of motion and other simple procedures as an extensional therapy service as instructed by the Therapist (with appropriate completed/demonstrated skills competency).
• Affixes and records patient observations.
• Assists patient in the self-administration of medication.
• Maintains patient safety by performing safety needs of patients and using equipment safely and properly (foot stools, side rails, etc.).
• Performs other duties as assigned.

Qualifications

• All required education is a minimum requirement. Higher level of education is acceptable.
• A High School Diploma is required.
• CNA - Certified nursing assistant and valid in State Driver's license is required upon hire.
• Clinical\BCLS - Basic Life Support certification is required within 90 days from start date.
• 2 years in acute care, nursing home, skilled nursing facility experience is required.
• Home care experience required per condition of participation in the last 2 years.
