Responsibilities
- Supports participants with activities of daily living, restorative and supportive care necessary to the preservation of their independence.
- Maintains a safe environment for PACE participants.
- Reports changes in participant’s condition and needs to the Personal Care Supervisor (or designee).
- Assists with participant activities in the Day Center, including meal service and clean-up.
Requirements
- High School diploma or equivalent required.
- Current License as Certified Nursing Assistant (C.N.A) in the state of Colorado required.
- A minimum of one year’s experience in working with the frail or elderly population required.
- Must have a valid driver’s license, proof of insurance and have means of transportation.

Schedule
Full-Time, Monday - Friday, 4 or 5 days per week; 8 or 10-hours shifts.
Pay
$19.50 - $23.50 per hour, based on previous work experience.

Senior Community Care of Colorado PACE (Eckert) provides healthcare and supportive services for individuals age 55 and older, helping them live safely, comfortably, and independently in their own homes and communities. As a Program of All-Inclusive Care for the Elderly (PACE) provider and the first of its kind in Western Colorado, our interdisciplinary teams deliver personalized, comprehensive care tailored to each participant’s needs.
