About the Role
The Certified Nursing Assistant (CNA) supports patients in their home by delivering personal care under the direction of a Registered Nurse or Therapist. The role focuses on helping patients maintain independence with daily activities, observing and reporting patient conditions, and documenting care provided.
Requirements
- State certification for Home Health Aide training program and competency, listed on the South Carolina state registry
- Valid SC driver’s license with a good driving record
- CPR certification
- One (1) year of personal care experience
Preferred Qualifications
- High School Diploma or GED Equivalent
Responsibilities
- Follows the care plan established by the RN or Therapist; assists patient/family as needed and notifies RN or Therapist when updates to the care plan are required
- Assists patients with personal care activities, including bathing, dressing, grooming, skin care, hair care, nail care, feeding, oral hygiene, and shaving
- Assists with toileting, bedpans, urinals, catheter care, and emptying catheter bags, if applicable
- Supports patients in using mobility aids (e.g., cane, walker) and adaptive equipment for personal hygiene
- Documents care performed in the electronic medical record according to the care plan
- Reports changes in the patient’s condition (physical and/or mental) and environmental concerns to the case manager
- Trains and assists family members with lifting, turning, repositioning, and transferring the patient
- Provides emotional support to patients and/or family members
- Maintains a safe, secure, and healthy patient environment by adhering to asepsis standards and policies
- Completes 12 hours of in-service training annually
- Performs other assigned duties as needed
