About the Role
Provide safe, quality, effective care to all patients in compliance with Hospice policies, procedures, and guidelines set forth by state and federal regulatory departments. Maintain patient confidentiality at all times and follow the patient plan of care established by the RN case manager.
Responsibilities
- Deliver patient care in accordance with Hospice policies and regulatory guidelines.
- Maintain strict patient confidentiality.
- Adhere to the patient plan of care established by the RN case manager.
- Travel extensively to patient homes in various conditions and environments.
- Provide nursing personal care while coping with family emotional stress and individual lifestyles.
Requirements
- High School Diploma or GED.
- One year of related experience with a working knowledge of standard practices and procedures.
- Certified Nursing Assistant (CNA) certification.
- Valid Driver’s License, reliable transportation, and valid automobile insurance.
- Healthcare Provider (CPR) certification (BLS Certification must be obtained within 30 days of hire).
- At least two years of certified nursing assistant experience, preferably one year in a Hospice setting.
- Ability to drive own vehicle and travel extensively during all seasons.
- Emotionally mature, stable, and able to work in various home environments.
- Good verbal and written communication skills.
- Ability to read, write, and follow written and verbal instructions effectively.
- At least 18 years of age.
- Neat, professional appearance and pleasant demeanor.
