CNA Coordinator - Hospice

Responsibilities

• Allocate resources to increase staff to patient ratios.
• Increase clinical visits while lowering clinician caseloads.
• Assist with staffing assignments for new admissions and scheduling.
• Respond to phone calls from all disciplines, family members and referral sources.
• Follow up with staff to provide support and education.
• Avoid missing patient visits by ensuring the daily schedule of patients is complete and updated in the database.
• Complete weekly reports and address them in a weekly meeting with the direct supervisor.
• Review and export all visits in the database, communicate with CNAs regarding their documentation and timecards, and report this information to the supervisor weekly.

Requirements

• Previous leadership/management and/or hospice experience.
• Excellent communication skills.
• Strong familiarity with computer applications and spreadsheets.
